How do I print my course certificate/provider card?

Please note: Your ONS Certificate for the following courses will now serve as your provider card.

  • ONS Fundamentals of Chemotherapy and Immunotherapy Administration
  • ONS Fundamentals of Chemotherapy and Immunotherapy Administration Renewal 
  • ONS/ONCC® Chemotherapy Immunotherapy Certificate™
  • ONS/ONCC® Chemotherapy Immunotherapy Certificate Renewal™


Follow these steps to access your course certificate and/or print your Provider Certificate. 

  • Log into your Learner Dashboard using your ONS Username and Password
  • Click on the Person Icon in the top right side of the screen to display the drop-down options
  • Click on My Transcript


  • Click on the Certificates tab  
  • Once you locate your course, you will have two options under Actions
    • View your certificate
    • Download your certificate 


Once you have completed the Evaluation within your course,  you can access your certificates and provider card by visiting myoutcomes.ons.org.

Log in using your ONS Username, which is your preferred email address listed on your account.

Enter your ONS password.

Click <span class="component"><i><b>Sign In</b></i></span>

Click <span class="component"><i><b>Transcript</b></i></span>

Click <span class=""><i><b>highlight</b></i></span>

This will open a new browser window. Click on the download to open your certificate and provider card.
