ONS offers a 30-day grace period following the expiration of your provider card. During that time, a $35 late fee will be added to your registration cost.
If your provider card has been expired for greater than 30 days, you are no longer eligible for the Fundamentals of Chemotherapy Immunotherapy Administration Renewal Course. To renew your card, you are required to register for and successfully complete the Fundamentals of Chemotherapy Immunotherapy Administration Course.
ONS reserves the right to deny registration to the Fundamentals of Chemotherapy Immunotherapy Administration Renewal Course based on provider card expiration date. If you are past your 30-day grace period due to an extended leave of absence from work, you are welcome to send supporting documentation to ONS Customer Relations for further consideration.
